Practice Area

We are pleased to welcome any new patient who lives within our practice boundary. Our catchment area is centred on Loxwood village, however our Practice boundary extends and covers all the local villages of Ifold, Wisborough Green, Plaistow, Kirdford, and parts of Dunsfold, Rudgwick, Cranleigh and Billingshurst.

If you would like to register as a new patient and are unsure if your home address is within our catchment area, please use the practice boundary tool below, email your full address and postcode to loxwood.txt@nhs.net or call our reception team for advice.

If you are visiting the area, you can access services by registering as Temporary Resident - please see the section below.

New Patient Registration Online

We are able to offer you an online registration process, this requires no trip to the surgery initially, but you may be asked to provide us with additional information and possibly attend a New Patient Medical with a practice Nurse.

Your main home residence must be within our practice boundary to register at our surgery, if you are unsure please contact us with your postcode. Please allow 7 working days for us to complete your registration process. If you need medical advice quickly please contact our reception team on 01403 755246. Once we have completed the registration process you will be a patient of Loxwood Medical Practice and can access all of our services.

Step 1: Start your registration with Loxwood Medical Practice by completing this online NHS form. 

Step 2: Please complete the new patient health questionnaire for adults and submit this information to us. This is because it can take a considerable time for us to receive your medical records from your previous GP practice. 

Step 3: If you would like to use any of our online services to book appointments, order repeat medication or access to your GP records, you will need to also complete this application form. You need to bring the completed form to the surgery along with photo ID and proof of address before a unique username and password are issued to you - this is to ensure security of access to your medical records. Alternatively you can access limited GP services via the NHS App/website but will still need to complete the application process for full functionality.

New Patient Registration in Person, by Post or Email

Our reception staff will be happy to talk you through the registration process.

These forms are available either from our reception desk or you can download via the links below, print out at home and return to us as soon as possible. Ideally it is preferable that you bring some form of identification in with you, such as a passport or driving license, so that we can see your full name and date of birth so that we can accurately register you and locate your medical records from your current GP surgery, but this is not essential.

If English is not your first language, please inform us. If you would like us to arrange a translator to be available during any consultation please let us know as soon as possible.

Step 4: Bring the completed forms with you to our reception team, send them in by post or email to the address on the form. (NB: Email is not considered fully secure as it passes through the internet, so if you have any concerns please use another method)

Register as a Temporary Resident

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

Repeat prescriptions whilst away from home can still be obtained if you are registered at an English GP practices. Your practice can send a prescription electronically to any pharmacy in England. Do not register as a temporary patient for this purpose but contact your usual practice instead.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
